of March of 1980.

Candidates:
Those foreigners who arrive at the national territory with the aim to be put under medical treatment in hospital prevailed, and with the obligation to leave the country, to more
to take, thirty (30) days after to have concluded his hospitalization or treatment.

- Medical certificate of Good Health sent within
the three (3) months previous to the date of presentation. (It must have date,
company’s signature and stamp with name, code and license of the doctor).

The Power will have to appear personally before civil employee of the National Management of Migration and Naturalization or with the Note of Personal Presentation before a Public Notary.

- Complete and general information of the checks that are contributed:
- Number of the Check,
- Name of the Bank,
- Date,
- Amount
- List and identify the documents that they enclose.
- Expose the foundations in fact and of right.
- Tax stamp of value of USD$4.00 by page.
- Background Record of the country of origin or of country where it has
resided interested last the two years. If the person interested is two or more
years old of it calls to account continuous in Panama is free to present this
document.
- Passport of the country of origin with a minimum validity of six months of each
one of the applicants.
- Photocopy of the passport of each one of applicants.
- Two (2) photos membership card size of each one of applicants.
- Sworn declarationon of Personal Antecedents.
- Certified Check or of Management by the amount of ONE HUNDRED DOLLARS (USD$100.00) in favor of the National Treasure.
- Medical Certification of the country of origin of interested where its incapacity is credited, diagnosis of its disease and the duration of treatment, which must be by a maximum period of (1) year. In the Certification it must be recommended medical treatment in Panama, to have the company’s signature and stamp with the name, code and record number of the doctor who it sends and to be properly added or authenticated by the Panamanian Consul in that country and by the Ministry of Outer relations in Panama.
- Letter of the hospital or medical center in Panama that will confirm that the foreigner will be or is hospitalized. This letter must contain the name of doctor who take care of it and ailment that is treating.
- Photocopy of the effective Membership card of the resident relative or photocopy of the effective personal Identity card of resident relative, authenticated by the Civil Registry.
- Letter of responsibility of the resident, relative, or designated tutor, in who this one takes responsibility economically of the foreigner, he affirms that he will Not represent load for the Panamanian State, and that commits a to repatriate it in case of being necessary.
- Proof of Economic Solution of the resident that can to be:
- Declaration of Rent, with its respective receipts of payment.
- Letter of Work with the last Card of the Social Insurance or the last Checkbook of the Check of Payment, for dependants public.
- Letter of Banking Reference.
- Personale identification document Retirement.
